Each Coach needs to please download, print and complete:
- Tournament Team Sign-Up Sheet
- Receive a Parent Consent and Insurance Waivers from each player.
Each player must have waiver signed by parent/guardian.
Tournament Team Sign-Up Sheet and Parent Consent and Insurance Waiver must be turned in at the same time
Pay fee of $135
